小児の仙骨麻酔質疑

質問（諏訪）：
小児のヘルニアなどに仙骨ブロックをされる方が少なくないと思いますが，これに関して２，３お教え下さい．
年齢の下限はどのへんですか．またそうする考えの論理や根拠は．
（私たちは，一応１歳以上，としています．根拠は薄弱です．）
中毒の報告はありますか．その条件とか注意点は？
噂では，仙骨硬膜外腔に炎症や線維化が起こる由です．（当然でしょうが）．それがどの程度のものか，どう対応すべきか，報告の論文を含めてお教え下さい．

研修医時代に派遣された獨協の越谷病院では，かなり盛んに行っていました．特に小児外科の鼠径ヘルニアや陰嚢水腫に対してです．もちろん，仙骨麻酔単独ではなく，GOF Slowで導入して，点滴を入れてから側臥位にして仙骨麻酔を行いました．
年齢の下限は，あまり意識していなかったように覚えています．私自身，生後24時間を経ていない新生児の鎖肛の人工肛門造設術と，生後18日の幽門狭窄に対するRamtedt手術に対して，仙骨麻酔を行ったことがあります．（後略）

仙骨硬膜外腔の炎症，線維化
Ｄａｌｅｎｓの教科書（Pediatric Regional Anesthesia,CRC Press ,1989）
では合併症として記載されていません．ただ，仙骨部からチュービングをすると感染の危険が高いので，禁忌とDalens は述べています．清潔な操作で清潔な局所麻酔のみをシングルショットで入れている限りは，大きな問題ではないような気がします＜私的見解＞．
